This means each client receives a personalized dementia wellness plan, 24/7 access to geriatric-trained specialists, and ongoing family communication — all at no cost for qualifying seniors.
Who qualifies for the GUIDE Dementia Care Program?
To qualify, individuals must have a confirmed diagnosis of dementia or Alzheimer’s disease, be enrolled in Medicare, and live at home or with a family caregiver. Our team helps families through every step of the eligibility screening and enrollment process, making it simple and stress-free.
Does the program provide support for family caregivers?
Yes. Family caregivers play a vital role in our approach. The program includes $2,563 annually in respite care funds to help caregivers take well-deserved breaks while their loved one continues to receive professional, compassionate care at home.
How long does it take to begin services?
After the initial eligibility screening and personalized care assessment, Medicare typically reviews and approves applications within 2-3 weeks. Once approved, clients can begin receiving care and support services right away.
